NEWARK, Del.--(BUSINESS WIRE)--“Control Your Heart for the Future,” a program developed byDelaware Physicians Care in collaboration with Christiana Care Visiting Nurse Association (CCVNA), has won a national award for innovation from Medicaid Health Plans of America. The award recognizes the program as an “overall best practice of Medicaid health plans in the industry for 2012-2013” and is one of six national awards. This award was the only one presented in the innovation category of the MHPA 2013 Best Practices Awards.
The program helps Delaware Physicians Care members who have congestive heart failure (CHF) to improve their quality of life by effectively managing their condition through education, self-monitoring, medication management and efforts to address their socio-economic needs. Members agree to participate in the program, receiving tele-monitoring services that recognize trends in vital signs that signal when CHF conditions are getting worse. When such signs occur, CCVNA nurses intervene, working with physicians to help members avoid in-patient hospital stays and trips to the emergency room.
